Endometriosis
70% of patients w/ pelvic pain.
Lesions MC found on ovaries, uterosacral ligaments, cul-de-sac
Black, dark brown, blue
tx: laparoscopy and ablation
Tx of Rectovaginal fistula following episiotomy with vaginal birth
wait at least 3-6 months for inflammation to subside before surgical repair
simple low fistula/some mid-rectovag fistula repair: endorectal advancement or anorectal flap technique
high fistula repair: transabdominal approach
permanent colostomy for elderly/unfit pts
W/u for abnormal uterine bleeding
Transvaginal US first to assess for any masses
Endometrial Bx or curettage
Tumor marker for granulose cell tumor of ovary
Inhibin
Non-epithelial ovarian cancer
Tumor marker for choriocarcinoma
HCG
Tumor marker for endodermal sinus tumor
AFP
Tumor marker for dysgerminoma
LDH & HCG
Tumor marker for epithelial ovarian cancer
CA-125
Indications for emergency C-section in pregnant trauma patients
Maternal refractory shock
Threat to maternal life from exsanguination
DIC
amniotic fluid embolism
Uterine injury or rupture
Fetal distress in viable fetus (23-24 weeks)
Unstable thoracolumbar spinal injury
