What classifies as failure to thrive?
inadequate calorie intake, calorie absorption, increased metabolism, defective utilization
S+S of Failure to thrive?
growth failure, withdrawn, feeding difficulty, avoids eye contact, minimal smiling
What is GER?
Gastroesophageal Reflux
regurgitation of gastric contents
What are GER complications?
failure to thrive, bleeding, dysphagia
GERD?
esophageal tissue damage
S+S of GERD?
cannot gain weight, irritable, N, chest pain, tooth erosion, apnea, bronchospasm, pneumonia, wheezing
Infant S+S of GER
spit up, vomiting, back arching, irritability, growth failure, resp. probs., apnea, excessive crying, cough
Nursing care for GER?
small more frequent feedings, thickened feeds, upright feeds, elevate HOB, NG feeds, burping
Medications for GER:
Histamine blockers to decrease acid production (cimetidine, famotidine, nizatidine, ranitidine)
PPI lower acid production (-zole)
Prokinetic agents to help empty stomach (reglan)
What is a Nissen Fundoplication
fundus is wrapped around the distal esophagus to create a anti-reflex valve
Cleft Lip?
during embryonic dev., problems with feeding, speech, ear infections, dental issues
Cleft Palate?
during fetal dev., inability to suck
Nursing Considerations for Cleft lip/palate?
45 degree angle for feeds, nipple/bottle type, abdominal distention, monitor for ear infections, delayed tooth eruption, impaired speech
