What are the steps of the nursing process?
ADPIE
Assessment, diagnosis, planning, intervention, and evaluation.
What is assessment?
Collect info about patient (subjective and objective). This is ongoing during all phases. Organize info by clustering, reporting, and documenting information.
What is diagnosis?
Problem r/t reason. Prioritize hypotheses (1st level- life threatening, 2nd level- urgent, 3rd level- important). Provides basis for outcomes and interventions.
What are the types of nursing diagnoses?
- Risk- describes response to health conditions/life processes that may develop
- Problem focused- describes response to health conditions that actually exist
- Health promotion- like nutrition and exercise, it enhances health behaviours
- Wellness- describes responses to levels of wellness that have readiness for enhancement
What is planning?
Occurs from 1st contact to discharge. Set a patient centred SMART goal. Pick out interventions for patient to accomplish goal.
What is implementation?
Initiate nursing interventions after reassess patient and organize resource and anticipate/prevent complications.
What is evaluation?
Were outcomes satisfactory and has change occurred in patient. Can also revise plan of care.
