impaired social interaction
insufficient or excessive quantity or ineffective quality of social exchange
characteristics: anxiety during social interactions, dysfunctional interaction with others, expresses difficulty establishing satisfactory reciprocal interpersonal relations. expresses discomfort in social situations
sexual dysfunction
a state in which an individual experiences a change in sexual function during the sexual response phases of desire, arousal, and/or orgasm, which is viewed as unsatisfying, unrewarding or inadequate.
characteristics: altered- interest in others, self-interest, sexual activity, sexual excitation, sexual role. decreased sexual desire
anxiety
an emotional response to a diffuse threat in which the individual anticipates nonspecific impending danger catastrophe, or misfortune
characteristics: behavioral/emotional, physiological factors, cognitive factors
ineffective coping
a pattern of invalid appraisal of stressors, with cognitive and/or behavioral efforts, that fails to manage demands related to well being
characteristics: altered- affective responsiveness, attention, communication pattern. destructive behavior towards others
caregiver role strain
difficulty in fulfilling care responsibilities, expectations and/or behaviors for family or significant others
characteristics: caregiving activities, caregiver health status, caregiver-care receiver relationship, family process
spiritual distress
a state of suffering related to the impaired ability to integrate meaning and purpose in life through connections with self, others, the world, or a superior being
characteristics: anger behaviors, crying, decreased expression of creativity, disinterested in nature, excessive guilt
ineffective airway clearance
reduced ability to clear secretions or obstructions from the respiratory tract to maintain a clear airway.
characteristics: absence of cough, adventitious breath sounds; altered respiratory rhythm. cyanosis
impaired skin integrity
altered epidermis and/or dermis
characteristics: abscess, acute pain, altered skin color, altered turgor, bleeding, blistering
disturbed body image
negative mental picture of one’s physical self.
characteristics: altered proprioception, avoids looking at one’s body, avoids touching one’s body, consistently compares oneself with others, depressive symptoms.
deficient knowledge
absence of cognitive information related to a specific topic or its acquisition.
characteristics: inaccurate follow-through of instruction: inaccurate performance on a test, inaccurate statements about a topic
acute pain
unpleasant sensory and emotional experience associated with actual or potential tissue damage.
characteristics: appetite change, diaphoresis, distraction behavior, evidence of pain using standardized pain behavior checklist for those unable to communicate verbally.
chronic pain
unpleasant sensory and emotional experience associated with actual or potential tissue damage last more than 3 months.
characteristics: altered ability to continue activities, anorexia, evidence of pain, facial expression of pain
nausea
a subjective phenomenon of an unpleasant feeling in the back of the throat and stomach, which may or may not result in vomiting
characteristics: food aversion, gagging sensation, increased salivation, increased swallowing, sour taste
risk for surgical site infection
susceptible to invasion of pathogenic organism at surgical site, which may compromise health
risk factors: alcoholism, obesity, smoking
risk for bleeding
susceptible to a decrease in blood volume, which may compromise health
risk factors: inadequate knowledge of bleeding precautions
