Serious injury
- requires hospitalization for more than 48 hours starting within 7 days injury was received
- fracture of any bone except simple fracture of fingers, toes, or nose
- sever hemorrhages, nerve damage, muscle or tissue damage
- internal organ damage
- second or third degree burns or burns effecting more than 5% of the body surface
Incident and accident reporting
Accident- between the time of boarding and disembark of an aircraft with the intention of flight:
- death
- serious injury
- aircraft receives substantial damage
Incident
- occurrence other than an accident associated with the operation of aircraft safety of operation
Death
Fatal injury that results in death within 30 days of the accident
Immediate notification
FFIDICOD
F- flight control malfunctions
F- failure of any internal turbine engine component that results in the escape of debris other than that out the exhaust path
I- inability of any crew member to perform normal flight duties as a result of injury or illness
D- damage to property, other than the aircraft, estimated to exceed $25,000 for repairs (including materials and labor) or fair market value in the event of a total loss, whichever is less
I- in flight fire
C- collision of aircraft in flight
O- overdue aircraft that is believed to have been involved in an accident
D- damage to helicopter tail or main rotor blades, including ground damage, that requires major repair or replacement of blades
- notification in writing must be given for an incident when requested
Reports and statements to be filed
Accidents: - within 10 days Incidents: - Immediate notification whenever requested by representative of board Overdue aircraft: - after 7 days if still missing
