NSAIDs Flashcards
NSAIDs are supposed to _______
Inhibit Cox enzymes & bad prostaglandins
Which type of drug has Anti-Platelet use? Aspirin, tNSAIDs, Acetaminohen, Celecoxib
Aspirin.
None of the others are anti-platelet
Which drug is not an Anti-inflammatory? Aspirin, tNSAIDs, Acetaminohen, Celecoxib
Acetaminophen
Which drug has no side effects on stomach, platelet, kidney, uterus, or clotting in vessels?
Acetaminophen
Which drug has a pro clotting side effect?
Celecoxib
COX1 in platelets makes…
Thromboxane
COX2 in platelets makes…
Prostacyclin
What are the actions of COX2?
Development Inflamation Reproductive Tissue Repair Fever Renal Fxn Pain
DIRT is FRee of Pain
What are the actions of COX1?
GI protection
Platelet Aggregation
Renal Fxn
How do tNSAIDs work in terms of COX1/2 action?
Inhibit COX1/2 and reversible
How does Celecoxib work in terms of COX1/2 action?
Reversible Inihbition of COX2
How does acetaminophen work in terms of COX1/2 action?
Reversible inhibition of CNS COX2
How does aspirin work in terms of COX1/2 action?
Irreversible inhibition of COX1/2
Low dose is selective for COX 1
Increased bleeding risk occurs in side effects of COX _____ Inhibitors in _______ cells
COX 1 in Platelets
Increased clotting events occurs in side effects of COX _____ Inhibitors in ________ cells.
COX 2 in endothelial cells
What are the contraindications of tNSAIDs?
- Old people
- Stomach ulcers
- Concurant glucocorticoid use
Which drug and concentration is the COX1 selective inhibitor?
Low Dose Aspirin
Which tNSAID is best for patients with GI problems?
Ibuprofen
Inhibits COX2 more than COX1
GI-Ibuprofen
Which tNSAID is best for patients with Cardiovascular problems?
Naproxen
Inhibits COX1 more than COX 2
“Cross my heart” NaproXen has a cross (X) and goes with CV problems
Which drug doesn’t decrease kidney function?
Acetaminophen because it ONLY inhibits COX2 in the CNS.
Who would be a good candidate for Celecoxib?
People who cant tolerate high doses of NSAIDs due to GI problems but need anti-inflammatory effects.
NOT FOR PATIENTS WITH CV PROBLEMS
How do you treat Aceaminophen OD?
N-acetylcystine
Why is aspirin OD unique? How do you treat it?
Originally presents as respiratory alkalosis. Later it is a metabolic acidosis.
Treat with a gastric lavage and NaHCO3.
What are prostacyclin’s effect on vascular smooth muscle and platelets?
Vasodilation and anti-clotting
What are thromboxane A2’s effect on vascular smooth muscle and platelets?
Vasoconstriction and Pro clotting
Describe the effects of leukotrienes on inflammatory cell function and pulmonary/vascular smooth muscle.
- Neutrophil Chemotaxis, aggregation and transmigration (LTB4)
- Inc Vascular Perm, Bronchoconstriction, vasoconstriction. Think of allergies. (LTC4 LTD4 LTE4 )
