What are NSAIDs and how do they work?
Non-steroid anti inflammatory drugs; They block peripheral pain by inhibiting prostaglandin synthesis.
What are antiinflammatory effects?
HELPR- heat, edema, loss of function, pain, redness
What is the COX enzyme?
Cyclooxygenase; enzyme is responsible for the conversion of arachidonic acid into prostaglandin.
What does the COX 1 enzyme do?
Protect stomach lining, regulates blood and platelets.
What does the COX 2 enzyme do?
Triggers inflammation and pain
How do NSAIDs work?
Inhibit COX enzymes and prostaglandins
How do salicylates work?
They inhibit prostaglandin with antiplatelet properties, as well as COX inhibiting properties. They treat the inflammatory process. (Aspirin)
What are some interventions with salicylates?
They have a short half life, NEVER give to peds or pregnancy, take at meals/with food, take fluids, assess for GI bleeds and DC prior to surgery.
What are some foods with salicylates?
Prunes, raisins, licorice, spices like curry powder and paprika.
What do para-chorobenzoic acid nsaids do and a contraindication? (and example)
They treat RA, OA, and Gout, but can irritate the stomach and are highly protein bound so they will displace other protein bound drugs so watch with that (Indomethacin (indocin))
What do phenylacetic acid derivatives treat and examples?
Treat short term post op pain. Ex: Toradol, Lodine
What are examples of propionic acids?
Ibuprofen and Naproxen
What are some side effects of NSAIDs?
Anorexia, n/v, diarrhea, rash, purpura, tinnitus, dizzy, lightheaded, anxious, confused, fluid retention, edema, GI bleed, blood dyscariasias, dysrhythmicas, nephrotoxicity, anaphylaxis
What drugs interact with NSAIDs?
Dilantin, sulfonamides, coumadin increase effects, ACE inhibitors interfere, avoid feverfew, garlic, ginger, ginkgo
