NHS DMS- what kind of service is it
it is an essential service
commissioned under the NHS Community Pharmacy Contractual Framework.
aims of this service (5)
Optimise the use of medicines whilst facilitating shared decision making
Reduce harm from medicines at transfer of care
Improve patients understanding of their medicines and how to take them following discharge from hospital
Reduce hospital re-admissions
Support the development of effective team working across hospital, community and primary care.
stage 1 DMS
receiving a notification that the referral has been received
perform a clinical check within 72 hours
compare what’s on patient medication record and discharge and compare them
check any outstanding prescriptions
clarification - must contact the hospital team
document this on the electronic system
stage 2 DMS
prescription is received
check the prescription against discharge letter
any issues - PMR
contact patient to tell them its ready
stage 3 DMS
patient invonvement
discuss with the patient (medication)
establish understanding, answer any questions
document on PMR and electronic DMS system
NHS trusts must ensure that appropriate patients are referred to community pharmacy within
24-48 hours
