What is the definition of a “drug history”?
taking history of a patient of what meds they’re taking at home/ before admission to hospital
Who can do a drug history?
pharmacist. technician. trainee pharmacist. BUT THEY NEED TO SIGN A COMPETENCE
What is medicines reconciliiation
seeing what the patient is taking before and seeing what is prescribed in the hospital and seeing any disceprencies
what is clinical checking
Making sure that the prescription is suitable for the patient
Who performs a clinical check

What does a clinical check ensure
Ensures:
Legality and legibility
Pharmacist ensures medicines are Safe and Clinically appropriate
what is accuracy checking
Making sure that what has been dispensed is accurate when compared to the prescription.
Why do we need hospital charts?
To provide a permanent record of the patient’s treatment with medicines whilst in hospital. (medicolegal document)
To indicate the patient’s sensitivity to medicines and allergy status
To facilitate the provision of the correct medicine from pharmacy.
To direct the administration of the medicine to the patient
how is a drug ordered in hospital pharmacy
Pharmacist will clinically check inpatient drug chart.
They sign the drug chart to confirm they have performed the clinical check.
A medication order form can then be signed by pharmacist or technician (with relevant training)
Medicines dispensed in pharmacy
Accuracy checked by technician/pharmacist
Medication delivered to ward
