If the pt is functioning bellow baseline note that & follow up

Progressive: Preserve function for as long as possible.
Maintnice & Med management are key focus along w. emotional status at pt. independence

Expensive
SNF
longer term services which are better for pts. w/ fatigue issues
Home Care
Equipment may be limited
IP rehab
best for pts. who need intensive daily therapy

Dx Chronicity Tx. setting Co morbididties (increase the potentioal amount of support thats needed) Support networks Wants and Needs

Physically taxing : Creative mvmt is benificial
