What is neurologically determined death?
- Irreversible loss of capacity of consciousness
- Irreversible loss of ALL brainstem functions
- INCLUDES capacity to breathe
What are the minimum clinical criteria for NDD
- Establish aetiology
- Deep unresponsive coma
- absent reflexes
- absent respiratory effort
- absent confounding factors
Which reflexes have to absent in NDD?
- No gag/cough reflex
- No motor responses, excludes spinal reflex
- Corneal responses
- Pupillary responses
- Vestibulo-ocular responses (caloric reflex)
What are some confounding factors in NDD?
- Unresuscitated shock
- Hypothermia - (<34)
- Severe metabolic disorders (glucose, phosphate, calcium, and magnesium), inborn errors of metabolism, liver or renal dysfunction
- peripheral nerve or muscle dysfunction or NM block
- Drug intoxications
Which drug intoxications would preclude NDD
- Alcohol, barbs, sedatives, hypnotics
Which drug therapeutic levels/dosing would NOT preclude NDD
- Anticonvulsants
- sedatives
- analgesics
What are the criteria for apnea testing?
- PaCO2>=60 and >=20mmHg rise above baseline
- pH<7.28
- COntinuously observe for respiratory effort
When should an ancillary test be performed?
when impossible to complete the minimum clinical criteria
drug unresponsive coma must be present
Established etiologic capable of causing ND
what is an example of an ancillary test?
- global absence of intracranial blood flow
When is it a conflict of interest for physicians declaring ND?
ANY physician with an association with the proposed recipient might influence judgement
List some brain death aetiology
- Trauma
- Intracerebral hemorrhage
- Hypoxia (drug OD, drowning, cardiac arrest post resus)
- Brain timor
- Meningitis
- Homicide or suicide
Describe steps in Apnea test
- Preoxygenate with 100% for 10 minutes
- Normalize acid-base status
- Disconnect from ventilator
- 100% O2 via trach catheter
- Observe for reap movement
PaCO2>=60, >=20 pH<7.28 no spontaneous resp, no resp reflex
What are some physiologic changes that happen during NDD
- Central diabetes insipidus
- Vasodilation
- Apnea
- Hypothermia
- Hypothyroidism
- Hypoadrenalism
What are the key differences between NDD and PVS`
- PVS: spontaneous ventilation
- PVS: maintain CV stability
- PVS: sleep/wake cycles
- PVS: months to years
- Brain death - hours
After declaring death
- Offer organ or tissue donation
2. Disconnect ventilator, complete death certificate , offer autopsy
