Impaired ability to orient toward one side
- PRESERVED motor and sensory functions

How do you test for hemineglect
- Double simultaneous stimulation - visual or tactile
- Line bisection
- cancellation test
If you have a LEFT parietal lesion - look for
DOMINANT hemisphere -
GERSTMANNS SYDROME
Characteristic features of gerstmann’s
- acalculia
- agraphia without alexia
- Finger agnosia
- Impaired left/right discrimnation
- ideational apraxia
Other tests of cortical sensory fxn
- Graphesthesia
- 2-point discrimination
- Point localization
Hemineglect OFTEN indicates a
RIGHT parietal lesion
- right parietal lobe attends to both hemispheres - dont see neglect with a left stroke
If it’s not in the right parietal lobe
- Right prefrontal
- Right cingulate gyrus
- Right striatum, posterior internal capsule, and thalamus

What is apraxia
inability to perform movements in response to a command - often skilled or learned movements
ideomotor vs ideational
ideational is sequence problem
How do you lose praxis?
- Disconnection/destructtion of DOMINANT HEMISPHERE association areas
- NOT hemiparesis, hemisensory deficits, cerebellar, or aphasias
Differential diagnosis of Ideomotor apraxia
- 80-90% non fluent primary progressive/corticobasal syndrome
- 70-80% Alzheimer’s disease
- 70& PSP
- 28-57% STROKE
- 25% Parkisons
