Sudden onset in seconds or minutes
Red flag for headache - Hemorrhage or infection
“first or worst headache”
Red flag for headache - Mass, AVM
Focal neuro symptoms

Fever

change in personality, mental status, LOC
Papilledema
Swelling of the optic disc due ot increased ICP. Almost always bilat
When to image with a HA
- Red Flags
- Change in pattern, frequency, or severity of HA
- Worsening of HA despite therapy
- Abnormal exam
- Onset of HA with exertion, cough, sex
- New onset > 40 yo
Duration of migraine
4-72 hours
Trigeminal Neuralgia
sudden, brief, unilateral facial pain
Distribution over one or more branches of CN V
Common in elderly
Tx Trigeminal Neuralgia
carbamazepine (Tegritol)
Tx of bells palsy
Steroids and acyclovir
Vertigo - Peripheral Etiology
benign positional vertigo
Meniere’s disease
acoustic neuroma
otitis media
Vertigo - Central Etiology
migrainous vertigo, brainstem ischemia, MS, chiara malformation
Sx of Parkinson’s disease
rest tremor, rigidity, bradykinesia, and gait disturbance
Dx of Parkinson’s
Dopaminergic therapy
