On a CBC the size of RBCs is described as?
MCV (mean corpuscular volume)
On a CBC the hemoglobin content “color” of RBCs is described by the?
MCH (mean corpuscular hemoglobin)
Hgb range
11-15
Hct range
33-45%
RDW means?
The degree of variation in RBC size
Abnormal RDW
> 15%
serum iron
measure of the iron in circulation
serum ferritin
measure of iron in storage
reticulocyte count
indicates the ability of the bone marrow to produce RBCs (body’s response)
peripheral smear
a visual description of the RBCs
TIBC (transferrin)
Total iron binding capacity (think egg carton)
Iron deficiency anemia
microcytic hypochromic anemia
How long should iron be replaced for?
4-6 months
Anemia of chronic disease
mild to moderate normocytic or microcytic, normochromic anemia (think early death)
Other findings in anemia of chronic disease
Serum Fe low but TIBC and serum ferritin normal.
Thalassemia
miscrocytic, hypochromic anemia but normal RDW, serum Fe, TIBC, and serum ferritin
Tx for thalassemia
No inappropriate iron consumption
What anemia should be a joint screening
B12 and folate
Vitamin B12 deficiency anemia
Macrocytic, often normochromic, anemia
Neurological defects if B12 improves when?
5-10 days
petechiae purpura
Non blanching lesions common with B12 def.
Dx of thrombocytopenia
Platelet count <150,000
