Geriatric Screening - 5 items
Are you basically satisfied with your life? Do you often get bored? Do you feel helpless? Stay home rather than going out? Feel pretty worthless? 2 or more yes is positive
Labs for depression
CBC. lytes, glucose, BUN, creat, lives, TSH, b12, folate
Major depression
5 or more of the following symptoms present most of the day nearly every day for 2 consecutive weeks. At least one symptom must be depressed mood or loss of interest or pleasure.
SIG: E CAPS
Sleep issues Interest Guilt ENergy Concentration Difficulties Appetite Psychomotor agitation or irritation Suicidality
Examples of SSRIs
citalopram, escitalopram Fluoxetine Paroxetine Sertaline Fluvoxamine
Side effects TCAs
Can’t see
Can’t pee
Can’t spit
Can’t defecate
TIme frame for taking antidepressants
6-9 months
RUQ conditions
cholecystitis, right lower lobe pneumonia, acute hepatitis
RLQ conditions
appendicitis, ectopic pregnancy, ovarian cyst, diverticulitis, endometriosis, ureteral calculi
LLQ conditions
diverticulitis, RLQ diseases
LUQ conditions
gastritis, pancreatitis, MI, left lower lobe pneumonia
Murphy’s sign
cholecystitis
Psoas sign/Obturator test
appendicitis test
Rome III croteria - IBS
Recurrent abdominal pain or discomfort at least 3 days/month in the last 3 months associated with 2 or more of the following:
Relief with defecation
Onset associated with a change in frequency of stool
Onset associated with change in form or appearance of stool
Sx inconsistent with IBS
rectal bleeding, nocturnal or progressive abd pain, weight loss, anemia, elevated inflammatory markers, lyte disturbances
