Myocardial Sympathetic Innervation Imaging Flashcards
1
Q
what is the indication for myocardial sympathetic innervation?
A
evaluation of NYHA functional class II/III heart failure and left ventricular ejection fraction <= 35%
2
Q
what is the contraindication?
A
- interfere w/I123-MIBG that cannot be withheld for at least 24hr
- renal insufficiency
- Iodine allergy
3
Q
patient prep for myocardial sympathetic innervation?
A
- NPO after midnight
- Med interfere with catecholamines should be withheld 24hr ( antidepressant, antipsychotic, and calcium channel blockers
4
Q
what is the Rp and dose of myocardial sympathetic innervation?
A
I123-MIBG 10mCi
5
Q
what is procedure of myocardial sympathetic innervation?
A
- potassium perchlorate 400mg or lugol 100mg 1hr before Rp
- lie quietly 5min before inject
- Inject Rp 1-2 min
- image 15 post injection ANT
- SPECT images after static
- repeat 4hr delayed
6
Q
how to draw ROI in myocardial SYmpathetic Innervation?
A
- ROI around heart and mediastinum
- Define both lung apices, draw a vertical line between. a horizontal line to determine the most superior lung apex. the area with the lowest counts draw a 7x7 pixel ROI to determine the mediastinal
7
Q
I123 MIBG washout formula
A
([CD heart early -CD heart late]/CD heart late)x100
normal 10%
