necrosis occurs in full thickness of myocardium wall
-ST elevation myocardial infarction (STEMI)
non-transmural infarction
“subendocardial infarct”
vessel becomes reperfused or clot is lysed before full thickness necrosis can occur
- non-ST elevation myocardial infarction (nonSTEMI)
multifocal microinfarction
occurs when small microvessels are occluded
Infarct in LAD
anterior wall of L ventricle near apex
Infarct in RCA
- inferior posterior wall of L ventricle
- inferior posterior wall of R ventricle
Infarct in Circumflex
Lateral portion of L ventricle, not at apex
Cardiac specific troponin protein
released after myocardial damage
- elevated after 3-12 hrs
- peak at 24 hrs
- back to normal after days
Creatinine Kinase MB
MB is sensitive but not specific to cardiac muscle damage
- elevated after 3-12 hrs
- peak at 24 hrs
- back to normal in 48 hrs
Familial Hypercholesterolemia
Autosomal Dominant inheritance!
- increased total cholesterol and LDL
