What is myasthenia gravis?
A disorder of neuromuscular transmission
What causes MG?
Binding of autoantibodies to components of the NMJ
Describe the pattern of muscle weakness?
Worse on exertion, gets better with rest
What other manifestation will most patients have at some point?
Ocular
Where might weakness be seen?
Small muscles of the hands, deltoid and triceps, bulbar muscles, mastication muscles
What might occur?
Seizures
What antibodies would you look for?
ACh receptor antibodies
What organ would you test?
Thyroid
Describe a crushed ice test
Ice applied to ptosis for 3 minutes, improves in MG
How do you manage MG?
Acetylcholinesterase inhibitors
Give an example of an acetylcholinesterase inhibtor?
Pyridostigmine
What are some alternatives to pyridostigmine?
Methotrexate, ciclosporin, tacrolimus
What drugs can exacerbate MG?
Ciprofloxacin, azithromycin, propranolol, atenolol, verapamil, lithium, statins, chloroquine, prednisolone
