What is the cauda equina?
The nerve roots caudal to spinal root termination
What can cause cauda equina syndrome?
Herniation of lumbar at L4/5 or L5/S1, tumours, trauma, infection, late-stage ankylosing spondylitis, postoperative haematoma, sarcoidosis
How quick is onset?
Sudden - hours
How does CES present?
Saddle paraesthesia, bladder/bowel dysfunction, sexual dysfunction, motor problems, lower back pain, bilateral LMN weakness, absent ankle reflex
What is CES?
A medical emergency
What would you find on a rectal exam?
Loss of anal tone/sensation
What imaging would you do?
MRI of spine
What management is essential?
Surgical decompression
What should you do to the spine?
Immobilise it
How else would you manage it?
Anti-inflammatory agents, antibiotics if infection, chemo if indicated
