What are the most common causative pathogens?
Meningococcus, pneumococcus
How does meningitis present?
Meningism, fever, headache, vomiting, nausea, lethargy, irritability, muscle pain, chills/shivering, sore throat, non-blanching rash, raised ICP
What are signs of meningism?
Neck stiffness, photophobia, Kernig’s sign
What are signs of shock?
Prolonged capillary refill time, DIC
What main tests would you do?
Blood cultures, LP, Throat swabs, ABG
What might a LP look like in bacterial meningitis?
Turbid, very high WBC, glucose <1/2 of plasma, high protein
What might a LP look like in viral meningitis?
Clear, lymphocytic predominance, glucose >1/2 of plasma, normal protein
What are some differential diagnoses?
Malaria, encephalitis, septicaemia, dengue, typhoid
When should you start antibiotics?
As early as possible
Who should you give prophylaxis to?
Household contacts in droplet range
What are some potential complications?
Shock, elevated ICP
Is acute meningitis a notifiable disease?
Yes
What can cause bacterial meningitis?
E.coli, listeria, HIB, S. pneumoniae, Klebsiella, TB
What can cause viral meningitis?
Mumps, herpes zoster, HIV, influenza, measles, enteroviruses
What are risk factors for meningitis?
Immunosuppression, smoking, CSF shunts, DM, IVDU, adrenal insufficiency, malignancy, SCD, crowding
