Presentation
Bilateral fatiguable ptosis - worse on up gaze
Variable strabismus and diplopia
Bilateral facial muscle weakness
WEakness of the neck muscles
Weak voice with nasal twang
Proximal weakness with fatiguability
Pathophysiology
Autoimmune disorder against nicotinic AChR
Investigations
Anti-nAChR
Anti MuSK
Drugs exacerbating MG
Penicillamine, aminoglycosides, fluoroquinolones, macrolides, beta blockers, calcium channel blockers, lithium, phenytoin
Lambert Eaton myaesthenic syndrome
Paraneoplastic
Attacks pre-synaptic voltage gated calcium channels
Management
Anticholinesterase - pyridostigmine
Steroids
Steroid sparing agents - azathioprine, ciclosporin
Thymectomy?
Can result in medication free remission
