anterior talofibular ligament
differences in different grade sprains in ankle
grade1-tear, can bear weight, no functional loss
grade 2-partial tear of ATFL and strech of CFL, more severe pain, some joint instability, significant pain w weight bear, loss of range of motion
grade3-complete tear of ATFL and CFL, partial tear of PTL. significant joint instability, loss of function, inability to bear weight
when should xr be done for ankle injury
if there’s bony tenderness of posterior edge or tip of the distal 6 cm of either medial or lateral malloelus
or if pt unable to bear weight
if there’s bony tenderness over navicular bone (medial foot), base of fifth metatarsal (lateral midfoot)
management of ankle sprains
PRICE: protect, rest, ice, compression, elevation
exam for joint injury
inspection palpation range of motion (passive, active) strength neurovascular status
when to do xr for knee injury
age 55 or older isolated patella tenderness tenderness of head of fibula inability to flex knee to 90 inability to bear weight for four steps (regardless of limping)
when should range of motion exercises be started after injury in pts with sprains and strains
48-72 hours
Ottawa Ankles rules apply to
nonpregnant adult pts w normal mental status, who don’t have other painful injuries, and seen within 10 days of injury
