how to approach asymptomatic microscopic hematuria?
begin w repeat urinalysis
if condition persists, should get imaging studies of both upper and lower urinary tract. also send urine for cytology and culture
upper tract imaging-IV pyelogram or CT
lower tract imaging-cystoscopy
microscopic hematuria
presence of three or more RBCs per HPF on two or more properly collected urinalyses
urologic causes of hematuria
tumors calculi infections trauma BPH
urologic hematuria is distinguished from other etiologies (glomerular or reanl) by
absence of proteinuria, dysmorphic RBCs, and erythrocyte casts
initial finding of hematuria by dipstick method should be confirmed by
microscopic evaluation of urinary sediment bc the dipstick is limited in specificity
presence of RBC casts and dysmorphic RBCs is suggestive of
renal glomerular disease
interstitial nephritis is suggested by presence of
eosinophils in urine
gross hematuria-approach?
full work up—urinalysis, urine culture, imaging of upper urinary tract by CT scan
tender or boggy prostate
diagnosis of prostatitis
