Muscle energy
Direct technique - patient placed into restrictive barrier
patient actively uses muscles on request to return to neutral
Postisometric relaxation
Engage restrictive barrier
Patient contracts against counterforce -> isometric contraction.
As muscle contracts, internal connective tissues stretch
Golgi tendon organs sense change in tension -> reflex relaxation of agonist muscle fibers.
passively stretch into new restrictive barrier
Reciprocal inhibition
contracting antagonist muscle -> reciprocal inhibition reflex arc - agonist muscle then forced to relax
Joint mobilization using muscle force
ME restores normal ROB of joint using muscle contraction
Ex. using hip flexors to tx anterior innominate SD
Oculocephalogyric reflex
EOM contraction to reflexively effect cervical and truncal musculature
Respiratory assistance
ME of voluntary respiratory motion to restore normal motion
Crossed extensor reflex
Crossed extensor reflex achieves muscle relaxation
extremity severely injured or not assessable to direct manipulation
ex. Contraction of right biceps produces relaxation of left biceps and contraction of left triceps
Upper T spine T1-4 ME
Seated position
use head and neck as lever to induce motion
engage restrictive barrier using head
instruct patient to straighten head against resistance
Hold 3-5 seconds, engage next barrier, etc
Lower T spine/lumbar ME
Seated position
hand on side of PTP grabs neck, other hand holds elbow
Type 1 - under over
Type 2 - over over
Engage restrictive barrier, activate by straightening body against resistance
Inhalation rib Dysfunction ME
supine
Treat lowest rib in group
flex up for pump handle
side bend for bucket handle while pt reaches for knee on affected side
Resist respiratory motion
Exhalation rib dysfunction ME
treat top rib
Supine with forearm on affected side across forehead with palm up.
Grasp key rib posteriorly at rib angle
Inhale deeply and hold breath while applying inferior traction on rib angle
Rib 1: raise head toward ceiling
Rib 2: head turned to 30 degrees away from dysfunctional side and lift toward ceiling
Rib 3-5: push elbow of affected side toward opposite ASIS
Ribs 6-9: push arm anterior
Ribs 10-12 - adducts arm
Forward sacral torsion ME
Axis side down, patient in modified sims
flex hips until motion felt at lumbosacral junction
Drop legs off table to induce sidebending and engage oblique axis
pt lifts legs towards ceiling against resistance
Backward sacral torsion ME
lateral recumbent w/ face up, shoulders against table
Axis side down
flex hips until motion felt, legs off table
lift toward ceiling against resistance
Posterior radial head ME
supinate to initial resistance, monitor radial head
pt pronates against resistance
engage next barrier
Anterior radial head ME
pronate to initial resistance, monitor radial head
pt supinates against resistance
