What is myelin?
An electrical insulator
Composed of protein and sphingomyelin lipids
As an extension of plasma membranes from Schwann cells and oligodendrocytes
Which facilitates faster saltatory conduction in myelinated neurons
While allowing smaller size and low energy consumption of neurons
Myelination occurs until 10 years old
What is multiple sclerosis?
Most common autoimmune inflammatory demyelination disease of the CNS
Leading to terminal Axonal degeneration
Presents with multifocal neuro-dysfunction disseminated in time and space
- optic nerve - optic neuritis
- brainstem - CN palsy
- cerebellum - TEN signs
- spinal cord - transverse myelitis
As a relapsing and remitting course (RRMS) with complete recovery
Which becomes secondary progressive with retained cumulative dysfunction (SPMS)
Young adults, Females > males, in higher latitudes
What are the DDx for MS?
Neuromyelitis optica (NMO)
-No plaques on MRI
