what is a Multi-disciplinary team ?
A team in which health and care workers from different professional backgrounds, and with different work roles, plan, implement and monitor an individual’s care
What can happen when agencies do not work together?
Chance of a vulnerable, at risk individual, to slip through the net
Why is it important that different agencies share information about an individuals care?
-These teams may not include only the health and care workers but also voluntary organisations e.g. Childline
-The emergency services, and the education services may also be represented.
-The work of a multi-disciplinary team ensures a holistic approach is taken to planning and implementing a care plan.
-Heath and care professionals not only provide their specialist knowledge but will also see this within the context of the wider needs of the service user
