How to write Control drug prescription for Class A, B & specified class C control drugs?

A

The prescription must be handwritten by prescriber
On a H572 form- a triplicate prescription form or on a paper generated from approved system=NZePS
But Class C would be in print or handwritten in a regular prescription form.

A

Every prescription has a prescription no on top which is for council or PSNZ if any prescription gets stolen then they inform pharmacy that this Prescription with the no has been stolen and do not prescribe medicine.

It has 3 copies 
•	1st copy is for our pharmacy top copy
•	Yellow page goes to health pack for claiming that we get paid for dispensing
•	And the red copy goes to Govt Medsafe.
•	Remember to stamp all three copies.

What we did during COV19 with control drugs?

A

Prescription is generated via approved NZePS and printed with barcode on a separate piece of paper.
• Signed by the CD prescriber
• We need a downloaded form that we can scan or enter the barcode manually
• If the prescription needs to be amended than a handwritten amendment is not acceptable, we need a new amended prescription.
• If a prescription emailed or faxed than we still needed a signed with ink prescription.

For prescription need to be maintain the following

  • Every prescription has to be signed physically handwriting
  • Legible and indelible
  • Dated on which it was signed
  • Stamped control drug prescriber
  • Set out the surname with initial of the firs name and address of the person to whom the controlled drug in intended to be administered or person who has custody of the animal to which the controlled drug is intended to be administered
  • Person under 12 years must need age months and years
  • If dentist prescribing the prescription, then mention for “dental treatment only”
  • If midwife prescribing “for midwifery use only”
  • In veterinarian “for animal treatment only”
  • The name of the drug needs to supply
  • Cannot use any other short form or abbreviation other than those abbreviation is listed in the pharmacopeia.
  • Indicate the total amount of drug that may be sold or dispensed on the one occasion. i.e repeats
  • Dose and frequency of the dose with direction like external and direction of use
  • If CD prescriber prescribe an unusual dose, then underline them and initial them to emphasised by being underlined.

A

Pharmacist and a person with authority under immediate supervision of pharmacist.

Prescription expiry and repeats in exceptions?

A

A medical practitioner or nurse practitioner for class A drug or Class B CDs can indicate in the prescription the how many intervals they want to dispense for the patient 7 days or in 2 occasions etc.so the 2 occasion cannot be not more than 7 days after the date of prescription and 2nd interval not more than 7 days after the termination of the interval.

Midwife exception prescription expiry?

A

A midwife who sign a prescription for CDs will direct on the prescription that the drug be supplied on 2 occasions with 2 specified intervals.
The first occasion being Not more than 4 days after the date of the prescription
The second occasion Not more than 4 days after the termination of that interval

Prescription expiry and repeats exceptions to baseline rules for CDs?

A

If a prescriber wants to dispense his patient daily CDs dispensed to in a circumstance for his patients’ safety than prescriber can write that prescription but must not exceeds more than 1 months and we have to dispense the way it has been written. if they want every 2 days, we have to give every 2 days

Class C must be supplied not more than 6 months after the date of the prescription

A

C category control drugs if they do not have any instruction then we will dispense monthly supply with 2 repeats similar like a NON stat medicine. Not more than 3 occasions which unless specified.

Phone and oral prescriptions for CDs?

A

Prescriber personally know the pharmacist and then Can phone through or by oral in case of emergency and require a original prescription within 2 business days and should indicate that it is confirmation of a phone prescription
Rule: REMEMBER a dentist and vets can’t call a pharmacist for phone or oral prescription request to a pharmacy.

Verification of prescription for new prescriber?

A

If we are dispensing from an unknown source like a new prescriber than we need to first Recognise their signature and satisfied that the signature is genuine. It can be done by phoning them and ask about the prescription has been written by that name. or by chaking their registration no that they are legitimate for dispensing by law.

How to supply a CD prescription?

A

Only give to the patient whom it been prescribed for or deliver them to home.
Unless we have a written authority from the patient.
Or we can be satisfied that the person who is collecting the prescription is a caregiver.
If someone come to pick the CDs and not authorised, then we might say that you cannot lawfully CD of your possession.

What Records needed for CDs?

A

In CD registered, Record must be entered by the end of Next business days and do not obliterate/cancel/alter any entry.
Stocktake needs to be done twice a year 30 June and 31 December.

How to store CDs in the pharmacy?

A

Locked cupboard/ compartment made of metal/ concrete approved by the Medsafe.
Must be fixed to the premises i.e. bolted to the ground
Key must be securely kept when not being used
