Lips
symmetrical at rest and while moving. Smooth and pink with minor Fordyce spots on vermillion border.
Buccal mucosa
pinkish red, smooth and moist; whitish yellow Stensen’s duct
Gingivae
slightly stippled and pink; tight margin at each tooth; no swelling, bleeding or lesions.
Teeth
ivory, firmly anchored, premolars and canines interlock fully
Hard palate
whitish and dome-shape with transverse rugae; no nodules.
Soft palate and uvula
(depress tongue and tell patient to say ah) soft palate rises symmetrical and uvula stays midline with phonation
Tonsils
blend into pink color of pharynx, no redness, enlargement or exudate
Posterior pharyngeal wall
smooth glistening, pink mucosa with small blood vessels.
Tongue
midline, no fasciculation (twitching); dull red; moist and glistening with papillae and small rugate. No swelling, ulcers or leukoplakia.
Floor of mouth
smooth and pink
Hypoglossal (CN XII)
(Have patient raise tongue to nose, down to chin and side to side) able to perform tongue movements with steady firm pressure.
