Extraocular muscle function
(hold patient’s chin and ask to look at finger while performing six cardinal fields of gaze test) smooth full coordinated movement of eyes bilaterally. This also tests oculomotor (CNIII), trochlear (CNIV) and abducens (CNVI)
Peripheral vision
(use confrontation test) intact
Accommodation
constriction with near objects. Dilatation with far objects
Corneal light reflex
(have patient look at near by object, shine light to bridge of nose) light reflected symmetrically form both eyes
Ophthalmoscopic examination (turn light off) Lens
no cloudiness or opacity in lens.
Ophthalmoscopic examination (turn light off) Red reflex/optic disk
(Have patient look at distance use ophthalmoscope and shine light to pupil from 12 inches and move towards patient) “As I get close, I focus on the red reflex and I will look at her optic disk, I have to be close enough to see through a small un-dilated pupil
Ophthalmoscopic examination (turn light off) (place thumb on forehead and rest ophthalmoscope on thumb) Fundus
fundus is yellow no lesions or hemorrhages;
Ophthalmoscopic examination (turn light off) (place thumb on forehead and rest ophthalmoscope on thumb) Blood vessels
minor venous pulsation, no nicks or tortuosity;
Ophthalmoscopic examination (turn light off) (place thumb on forehead and rest ophthalmoscope on thumb) Optic disk
optic disc is yellow with sharp well define margins especially on temporal region;
Ophthalmoscopic examination (turn light off) (place thumb on forehead and rest ophthalmoscope on thumb) Macula
macula is yellow surrounded by deep pink.
