Mood Stabilizers and Antipsychotics Flashcards
MOA of Carbamazepine
An anticonvulsant. Blocks voltage-gated sodium channels, decreasing the kindling of neurons
Uses of Carbamazepine
• Trigeminal neuralgia
• Bipolar disorder
o Treats acute manic episodes, mixed episodes
Side effects of Carbemazepine
Rash CNS: dizziness, slurred speech, ataxia Sedation Leukopenia Agranulocytosis/ aplastic anemia (monitor CBC while on treatment)
Drug interactions of Carbamazepine
Inducer of CYP450; decreases efficacy of oral contraceptives
MOA of Antipsychotics
Dopamine (D2) blockage. Also have significant anticholinergic and antihistaminic effects.
Differentiate the two types of Antipsychotics
Can be high potency/low dose (conventional AP’s) or low potency/high dose (2nd-gen AP’s).
The conventional AP’s (high potency) have stronger D2 blockage than the 2nd-gen AP’s.
★First line treatment for Psychotic Disorders = 2nd-gen antipsychotics
What are the side effects of Conventional antipsychotics?
- More Extrapyramidal Symptoms (EPS)
- Sedation, dry mouth, constipation, and hypotension – these are due to less histamine, anticholinergic, andregenic effects
What are the side effects of 2nd generation antipsychotics?
- Less Extrapyramidal Symptoms (EPS)
* More histamine, anticholinergic, andregenic effects.
Name the Conventional Antipsychotics
- Chlorpromazine
- Thioridazine
- Haloperidol
- Fluphenazine
- Trifluoperazine
- Perphenazine
- Pimozide
Name the 2nd generation antipsychotics
Risperidone • Olanzapine • Ziprasidone • Quetiapine • Aripiprazole • Paliperidone • Lurasidone • Asenapine • Clozapine
Which antipsychotics are best used for relapse prevention?
Decanoate, Haloperidol, Fluphenazine, Risperidone, Paliperidone, Olanzapine
Which type of antipsychotics are contraindicated in the elderly?
Anything anticholinergic; Use conventional antipsychotics.
Which drugs prevent suicidal ideations?
Lithium and Clozapine
