What are the mood stabilizers?
- Lithium (1st line for Bipolar)
- Valproate
- Carbamazepine
Indications for moods?
1st line for Manic Depression (Bipolar 1>2)
Do you typically use Lithium with Valproate or Carbamazapine?
Yes
MOA Lithium?
??? maybe inhibits inositol-phosphate signaling & neurotx stimulated
*Given as a salt so lost with sweating (dehydrated).. give Gatorade
Lithium PK?
- No metabolism; its an element
- Distributes to TBW & bone
- Cleared by kidney
Lithium DDI?
diuretics, NSAIDs
Lithium ADE?
Very narrow therapeutic window:
- Brain: tremor, ataxia, sedation
- Glands: edema, hypothyroid
- Kidney: nephrogenic DI
- Teratogen
- Skin: acne, psoriasis
- Heart: “sick sinus” syndrome (bradycardia-tachycardia)
Valproate MOA?
4 components:
- decrease Na channels
- decrease Ca channels
- Increase GABA
- Increase potassium conductance
Valproate PK?
inhibits UGT; increases its own concentration
Valproate DDI?
displaces phenytoin from plasma proteins
Valproate ADE?
heartburn & hepatotoxicity
Carbamazapine MOA?
Inhibit Na channels (prolong inactive state)
Carbamazapine PK?
induces CYP; decreases its own concentration
-CYP metabolism generates 10,11 epoxide (active metabolite)
Carbamazapine DDI?
- CYP inducer makes it react with many drugs
- decreased OCP efficacy is RED FLAG
Carbamazapine ADE? (4)
- Aplastic anemia
- Rash
- Ataxia
- Diplopia
