Mondays quiz Flashcards
Adenosine - Classification
Endogenous nucleotide
Adenosine - Indications (2)
Narrow or wide complex tachycardia with heart rate >150bpm
Adenosine - Pregnancy Category
C
Adenosine - Contraindications (7)
Second or third degree AV blocks, SSS, atrial flutter, atrial fibrillation, ventricular tachycardia, hypersensitivity to adenosine
Adenosine - MoA
Directly acts on sinus pacemaker cells and vagal nerve terminals to decrease chronotropic and dromotropic activity without causing negative inotropic effects.
Adenosine - Pharmacokinetics
Onset- 5-20sec, Peak- 20-30sec,
Adenosine - Reactions (11)
Facial flushing, paresthesia, headache, diaphoresis, hypotension, chest pain, palpitations, SOB, nausea, lightheadedness, transient asystole (average 6 sec)
Adenosine - Dosage
Adult: 6 mg rapid IVP. If no effect after 1-2 mins, 12 mg rapid IVP. (*12 mg may be repeated in unaffected for a max dose of 30 mg)
Peds: 0.1 - 0.2 mg/kg rapid IVP; max single dose of 12 mg
Atropine Sulfate - Classification
Anticholinergic (inhibits parasympathetic)
Atropine Sulfate - Pregnancy Category
C
Atropine Sulfate - Indications (1)
Symptomatic Bradycardia
Atropine Sulfate - Contraindications (4)
Tachycardia, hypersensitivity, unstable cardiovascular status in acute hemorrhage and myocardial ischemia
Atropine Sulfate - MoA
Inhibits the actions of acetylcholine at post-ganglionic parasympathetic neuroeffector sites. These blocked vagal effects result in positive chronotropy and dromotropy without increased inotropy. Used to treat primarily bradycardias and organophosphate poisonings
Atropine Sulfate - Pharmacokinetics
Onset- 1-2mins, Peak- 5min, Duration- 30-45mins
Atropine Sulfate - Reactions (7)
Tachycardia, paradoxical bradycardia when pushed slowly or less than 0.5 mg, dysrhythmias, palpitations, anticholinergic effects, dizziness, headache
Atropine Sulfate - Dosage
Adult: Brady: 0.5mg IVP q 3-5 mins; max dose of 3.0mg or 0.04mg/kg
Peds: 0.02 mg/kg IVP minimum dose of 0.1 mg. Max dose of 0.5mg for child and 1.0mg for a adolescent
Amiodarone Hydrochloride - Classification
Class III anti-dysrhythmic (potassium channel blocker)
Amiodarone Hydrochloride - Pregnancy Category
D
Amiodarone Hydrochloride - Indications (5)
Stable wide complex tachycardia; V-fib; Pulseless; V-tach; *May be ordered in Junctional and Multifocal Atrial Tachycardia
Amiodarone Hydrochloride - Contraindications (5)
Breast-feeding patients in cardiogenic shock, AV blocks, symptomatic bradycardia, sinus bradycardia with sinus node dysfunction, hypersensitivity
Amiodarone Hydrochloride - MoA
Decreases sinus automaticity, reduces the speed of conduction (-dromotropy), increases the refractory period of the AV node, prolongs the action potential duration in all cardiac tissue
Amiodarone Hydrochloride - Pharmacokinetics
Onset- 1-30min, Duration- 1-3hrs
Amiodarone Hydrochloride - Reactions
Hypotension, bradycardia, prolonged PR interval, pulmonary toxicity. May react with warfarin (Coumadin®), quinidine, digoxin, procainamide and phenytoin
Amiodarone Hydrochloride - Dosage
Adult: 150 mg SLOW IVP over 10 mins for JT, MAT, Stable wide complex tachycardia, may repeat once. 300 mg rapid IVP for pulseless VF or VT
Peds: Safety and efficacy not established
