During WEX, I saw how drs had long working hours
To conclude, I saw during my wex how healthcare pros
Managing multiple priorities and management strategies you’ve developed from it
During my semester 2 exams, I was fundraising for earthquake and supporting close family members, continuing regular translations for illiterate members. In order to complete each to high standards, I made a list of tasks and prioritised them. From this experience I developed ways to manage pressure by exercising outside to relieve stress and also keeping a diary to stay organised. This will help me as a medical student cope with high workloads and placements and also as a doctor so that I can continue to provide the best quality of care to patients even when under pressure.
Saw during wex in epsom hospital, I saw how consultant had multiple priorities, how stressful it was to manage a different patient every 10 minutes whilst still providing a high quality of care. Alongside this giving advice to registrars. I saw how the dr was able to take a small break to breathe between consultations.
Throughout my WEX I understood how a dr has to deal with stress and pressure throughout their shifts and also in the long run, in terms of managing healthy work life balance. So that you follow ethical principle of non maleficence to ensure no harm to patient. patient safety not compromised
By continuing to use the methods that I currently use, I believe that I will be able to achieve
Sharing MMI questions with a friend. Why did we ask you this
I would not share MMI questions. Not sharing maintains integrity, which is an important NHS value. They may do this again as a medical student and then as a doctor, this could impact the duty of patient care that drs are expected to uphold.
Talk to friend to see if there is a problem and if they need help. Professionalism
If there is a problem- support and be empathetic. Offer advice and point him in direction of help
Encourage to talk to uni about postponing date
Important because by acting in a professional way in a scenario like this then I would be upholding professionalism by following the rules which is a GMC regulation. Important in med to follow rules and regulations of med schl and gmc to maintain trust between patient and dr.
What different cultural experiences have you had. Why we asked you this.
Saw how dr interacted with patients of different backgrounds during consultations- patient with broken english came in with daughter, dr clearly explained test results using layman’s terms and made sure both understood. Saw importance of clear communication skills and empathy- meant patient felt fully informed about health and also trust dr improving patient dr relationship allowing dr to continue providing best quality of care (+autonomy)
- I implemented this into my own experiences
- E.g. volunteering at a foodbank where I interacted with people from different backgrounds by organising distributions of food packages. In a specific situation I came across a mother of 2 who spoke little english. In order to make sure that I was providing support that was beneficial, I adapted my communication to her and spoke slowly and emphasised important words. I actively listened to her responses and it was useful that I am a trilinguist as I was able to understand some words which further helped in building a rapport and providing her with the support that she needed. It was really important for me to be patient and understanding of her situation, making her feel comfortable to open up to me which allowed me to support her and signpost her to places useful to her.
In medicine, communication is really important when interacting with diverse range of people. To allow patients to make informed decisions, need to be able to explain to them regardless of background nhs core value. And also need to work alongside colleagues and mdt to provide best quality of care centred around patient. NHS core value and GMC regulation
Cultural competency - appreciate different cultures, backgrounds and beliefs. Not judgemental to colleagues and patients.
NHS core value is everyone counts- all about diversity and inclusion.
Asked me this to test my interpersonal skills and ability to work with people of different backgrounds
A patient that doesn’t want the COVID vaccine and has been researching online about it
Firstly I would explore what the reasons are
ICE
Then give them all the information they need to make an informed decision- patient has right to full autonomy on any treatment they receive
Actions based on 4 pillars of ethics
Autonomy- respect final decision
Beneficence- want to do most good, give leaflet or brochure, or write down reliable place to get info from NHS, bbc health. Help patient in making decision.
Non- maleficence- Informed consent, make sure patient is getting information from reliable sources. Work in partnership with patient and support them to make an informed decision
To conclude, it is important for me to take a non judgemental attitude and support my patient whatever decision they make
Arrange an appointment for another week -Continuity of care gp
ICE
Actively listen
Remain non judgemental and open minded
Give info
4 pillars
During WEX- saw toddlers parents refuse measles vaccine. Dr fully informed parents and ultimately gave them full autonomy.
Breaking bad news in a restaurant.
I would take passenger to a quiet and private area and let them take a seat and be comfortable
Get their perspective- see how much they already know, check their understanding
Let them know I have some bad news, to help them ease into the situation so its not sudden and they are prepared
Explain what happened in plain and simple language making sure that they understand me by speaking clearly. I would be completely honest and take full responsibility for my actions. (Integrity and honesty are really important attributes)
Remain silent and give them a moment to process the information. Apologise for my mistake
If they get angry, remain calm and composed respect their response, offer tissues if they get upset and remain empathetic with them by appreciating what they are going through.
Offer some solutions- Suggest to talk to your manager about fitting extra tables in if there is space, or offering compensation and a full refund. Explore the customers views on these.
As a doctor, errors can be made and will need to admit to those and deal with them tactfully with a degree of compassion. Act with integrity and honesty upholding gmc regulations.
Customer assistant
Research in the last century biggest impact
Imaging techniques- allows us to visualise inside body without using invasive techniques
Patient care- sped up diagnosis and also more accurate. Helped detect impact of diseases on the rest of body, e.g. metastases in cancer, 1 in 2 people in UK impacted by cancer in their life.
Patient safety- Less invasive, no need to operate on people, which could be a traumatic experience for vulnerable individuals such as elderly.
This means that quality of life for patients has greatly increased. E.g. ultrasound can identify genetic abnormalities in unborn babies, which means treatment plans can be put in place before baby is born, improving management of condition
Advantage on NHS- quicker treatment of patients and less complications with invasive treatment.
Research is important because it can open up more options for patients, help build rapport, can provide best quality of care.
I developed my research skills when I completed an isp, saw how important it was to fully evaluate effects of drugs on different areas of body and if benefits were enough to overcome the side effects
Commitment to care - NHS core value
Empathy
Teaching english small groups of asylum seekers and refugees in community sessions. Provided emotional support, understood and respected.
Acknowledge their feelings
