You are taking a history from a patient who has just informed you that they have diabetes. What are the top 10 questions to ask
A
Type - T1 vs T2
When - When were you diagnosied
Care - Do you see a GP or consultant for management, do you attend a diabetic clinic?
Fx - How often do you see them?
HbA1c - When did you last have your HbA1c, and what was your result?
Glucose - How often do you check your glucose? What is a typical reading for you?
Hypos - Have you had any hypos? When was last? How often?
Management - How do you manage your diabetes (diet, med, insulin injection, pump)
Opth: Do you regularly attend an eye clinic? When last? Any findings?
Complications - Neuropathy, nephropathy, skin changes
