majority of cases are idiopathic, but in around 10-20% a cause is found:
drugs: NSAIDs, rifampicin
Hodgkin’s lymphoma, thymoma
infectious mononucleos
Minimal change disease what symptom is rare?
Hypertension
Minimal change disease have normotension
In minimal change disease wht are the clinical features ?
Frothy urine - proteinurea
No hypertension
What type of proteinurea is seen in minimal change disease
Highly selective only albumin and transferin
Diagnosis of minimal change disease
Proteinurea > 3.5g/24h.
renal biopsy
normal glomeruli on light microscopy
electron microscopy shows fusion of podocytes and effacement of foot processes
Tx of minimal change disease
PREDNISILONE
And then cyclophosphamide
Prognosis ?
Prognosis is overall good, although relapse is common.
Usually having FULL recovery with later relapses
Roughly:
1/3 have just one episode
1/3 have infrequent relapses
1/3 have frequent relapses which stop before adulthood
