Mineralocorticoid steroid
- Increases Na absorption = high fluid retention
- Low anti-inflammatory effect
- Potassium excretion = prevents hyperkalaemia
Fludrocortisone
Used in neuropathic postural hypotension (diabetes) or adrenal insufficiency due to septic shock
Most potent
Anti-inflammatory effect of no clinical relevance
Hydrocortisone
- Significant activity
- Should not be used for long-term disease suppression
- Useful for short-term basis via IV for emergency (life-threatening asthma or thyrotoxicosis)
Why should hydrocortisone not be used long term?
Due to fluid retention.

Sodium + Water retention → Hypertension
Potassium loss → Hypokalaemia

Mineralocorticoid effects are negligible with the high potency glucocorticoids:
