Midazolam (Versed) Adult Dose

Answer 1

IV/IO route:

• 2 mg Repeat once after 5 min if still seizing
• Contact Base for more than 2 doses

IN/IM route:

• 5 mg Repeat once after 5 min if still seizing
• Contact Base for more than 2 doses, unless Excited Delirium Syndrome present, in which case up to a total of 3 doses may be given as standing order in order to rapidly sedate patient

Midazolam (Versed) Pedatric Dose

Answer 2

• IV/IO
  
  • 0.1 mg/kg
  • Max single dose is 2 mg IV
  • Repeat once after 5 min if still seizing. Contact Base for more than 2 doses.
• IN/IM
  
  • 0.2 mg/kg
  • Max single dose is 2 mg IN or IM
  • Repeat once after 5 min if still seizing. Contact Base for more than 2 doses.
  • CONTACT BASE before any consideration of sedation of severely agitated/combative child
