IV amoxicillin + gentamicin

IV flucloxacillin

What is the treatment for Staph epidermidis endocarditis and who gets it?
IV Vancomycin + gentamicin, PO rifampicin (when switching from IV to oral)
Prosthetic valve patients
What bacteria are associated with gut bound endocarditis and what is their treatment?
Enterococcus faecalis + Strep bouis
Amoxicillin
or
IV vancomycin + gentamicin
What is the cut off between mild to moderate and severe community acquired pneumonia and what is the treatment for each?
Mild/mod = CURB65 0-2 = amoxicillin
Severe = 3-5 = Co-amoxiclav + doxycycline
Severe + ITU = Co-amoxiclav + clarithromycin
What is the treatment for non-severe and severe hospital acquired pneumonia?
Non-severe: amoxicillin + metronidazole
Severe: amoxicillin, metronidazole + gentamicin
What are the first and second line treatments for a COPD exacerbation?
First: amoxicillin
Second: doxycycline
What is the differentiation between severe and non-severe C. diff infection and what is the treatment of each?
Non-severe (diarrhoea without other symptoms): PO metronidazole
Severe (Alb <3g/dL, WBC >15000 + abdo tenderness): Vancomycin + metronidazole
How is peritonitis or any other abdominal infection treated (include if penicillin allergy) and what is the step down treatment for these?
Normal: IV amoxicillin, gentamicin + metronidazole
Penicillin allergy: vancomycin, gentamicin + metronidazole
Step down treatment: co-trimoxazole + metronidazole
What is the treatment for spontaneous bacterial peritonitis in mild and severe disease?
Mild: PO co-trimoxazole
Severe: IV piperacillin
