Early onset sepsis

A

Organisms: GBS (40%, most common), Ecoli (30%, highest mortality, mostly preterm)

  • others: strep viridans, strep pneumo, staph aureus, enterococcus

Timing: onset <72 hours in NICU, or <7 days in term

  • usually present within 6 hours

Transmission: vertical

RFs: low maternal GBS immunity, PROM > 18hr (1% risk), chorio (4%)

Tests: Total WCC poor PPV, BC (high conc in neonates), CRP triggered by IL-6 (rise within 6 hr and peak 24 hr) cut-off 10

Urine: not necessary as EOS UTI most likely secondary to renal seeding

LP: 38% meningitis negative BC
