Chicken pox (primary infection), shingles (aka zoster) (reactivation)=
VZV latency location
Dorsal root ganglion (sensory neurons)
VZV clinical presentation of primary infection
Fever followed by itchy rash (starts on scalp, trunk, spreads to limbs, but limbs don’t end up as heavily affected)
VZV clinical presentation of reactivation
Shingles: Sudden onset pain in thoracic or facial dermatome. vesicular rash distributed in a dermatomal pattern. Rash may last 2-4 weeks. Pain can last much longer than rash (postherpetic neuralgia)
Treatment of shingles
Acyclovir and steroids, vaccine can lessen or prevent symptoms
Treatment of chickenpox
Symptomatically: itchness, aches
Disease caused by HHV-6/7
Roseola (aka exanthem subitum, aka 6th disease)
Roseola clinical presentaiton
Mostly in kids 3-6 months old. Mild resp. illness –> high fever. After fever, rash on face and body (rash lasts 2-3 days). Rash moves from neck/trunk to extremities.
VZV pathogenesis
Infection –> viral replication in regional lymph nodes –> primary viremia (at day 4-6) –> viral replication in other organs –> secondary viremia –> infection of skin, appearance of rash (day 14)
