Cardiac conditions requiring SBE Prophylaxis
- Prosthetic cardiac valve or prosthetic material used for cardiac valve repair
- Previous infective endocarditis
- Congenital heart disease (CHD) : Unrepaired cyanotic CHD, including palliative shunts and conduits, Completely repaired congenital heart defect with prosthetic material or device, whether placed by surgery or by catheter intervention, during the first six months after the procedure, Repaired CHD with residual defects at the site or adjacent to the site of a prosthetic patch or prosthetic device (which inhibit endothelialization)
- Cardiac transplantation recipients who develop cardiac valvulopathy
Oral SBE prophylaxis dose: w/and w/o pencillin allegy
- Amoxicililin or Cephalexin : 2 g (adults) and 50mg/kg (kids)
- Clindamycin 600mg (adults) and 20mg/kg
- Azithromycin/clarithromycin:500mg and 15mg/kg
SBE prophylaxis unable to take oral medications
Ampicillin 2g IM/IV and 50mg/kg IM/IV
Cefazolin or cefriaxone 1g IM/IV and 50mg/kg
Cephalexin dosage for children and adults
Children >1 yo : 50mg/kg/day divided dose every 8 hrs.
Adults: 500mg every 6 hours
Clindamycin: dosage for children and adults
Children: 10mg/kg/day in 3-4 divided doses as hydrochloride
Adults : 300mg every 5 hrs
Metronidazole: what might it be rx for? adult and adolescent dosage
Periodontal disease, including necrotizing ulcerative gingivitis:
- 500mg every 8 hours for 8 days
Acetaminophen dosage for under and over 12 yo
Max doses?
12yo/adults: 400mg every 4-6 hrs prn
Max dose: 2.6g/day kids and 4g/day adults
Ibuprofen: under and over 12yo and adults? max dose?
12 yo : 200mg every 4-6 hrs MAX 1.2g/day
Adult 200-400mg/every 4-6 max 1.2g/day
Topical agents fro oral candidiasis?
Clotrimazole (10mg lozenge), miconazole (50mg buccal tablet), and nystatin (100kunit/mL suspension)
- Nystatin dosage: 1mL neonates, INfants 2mL and older 4-6mL swish 4xday for several minutes and swallow
Systemic agents for oral candidaisis?
Fluconazole and Ketoconazole (adults200mg/day, kids 5mg/kg day)
Topical agents for angular cheilitis?
Clotrimazole (cream 1%) Miconazole nitrate (cream 2%) nystatin (ointment)
