Meds Flashcards

1
Q

How easily does medication pass from plasma to milk in the first week of lactation?

A

Very easily as the spaces between lactocytes are wide spread allowing meds to easily transfer to milk. However, since baby is only taking small volumes of milk there may not be much concern for transfer to baby

Q

Drugs transfer into human milk if they are…

A
  1. Highly lipid soluble
  2. Attain high concentrations in maternal plasma
  3. Have a LOW molecular weight (<500)
  4. Are low protein binding
  5. Easily pass into the brain

Q

What is ion trapping in milk?

A

Due to lower pH of human milk (7.0-7.2) the ionic state of the drug changes and prevents its exit back into maternal circulation ( barbiturates and iodides)

6
Q

Warfarin, has a high maternal protein binding capacity. Would this medication be a risk for the infant?

A

NO. A high protein binding capacity allows milk to bind to plasma albumin and will stay in plasma circulation.

Q

Why would you recommend a Dr recommend a drug with a shorter half life?

A

Drugs with shorter half lives peak rapidly and then are eliminated form the maternal plasma- less exposure time to milk compartment. Urge mom not to breastfeed while e drug is at its peak level.

Q

Name 4 examples of medications that have a long half life and can build up in the infants plasma over time

A
  1. Barbiturates
  2. Benzodiazepines (tranquilizers ie. valium)
  3. Meperidine (demerol)
  4. Fluoxetine (prozac) (half life 216 hours)

Q

If you had a choice would you recommend drugs with a higher or lower protein binding capacity?

A

Higher. they are bound to the mothers plasma and not as easily transferred to the milk compartment

12
Q

Drugs move in and out of breast milk through passive diffusion – high to low- and with time may shift back the other way. Give an example

A

13
Q

Which medications those with a high molecular weight or a low molecular weight are a better choice for the breastfeeding mother.

A

High molecular wt limits movement into breast milk.
MW >500 does NOT transfer
Insulin MW> 6,000
Heparin MW 40,000

15
Q

True or false drugs that are active in the central nervous system usually have a low lipid solubility.

A

FALSE. The have high lipid solubility and therefore penetrate into breastmilk in higher concentration

16
Q

What are three factors that may affect bioavailability

A

Reduced absorption in GI tract
Poor GI stability due to acidity
High- first pass uptake by the liver

17
Q

Three medications with poor oral bioavailability

A

Gentamycin <1%
Insulin - destroyed in the gut
heparin - destroyed in the guy

18
Q

Give a general description of drug transfer into human milk

A

Drugs transfer into human milk if
1. have a high concentration in maternal plasma
2. Are small enough
3. Are non- protein bound
4. Are highly lipid soluble
5 are affected by oral availability in baby’s gut

19
Q

What are names of a few anti-depressants considered a better choice for the depressed mother?

A

Zoloft, paxil haloperidol, and reisperidone

20
Q

Are corticosteroids safe for the the mother infant dyad.?

A

Yes they generally do not transfer into milk.

21
Q

If a mother is undergoing treatment with radio isotopes how long would you recommend she pump and dump?

A

As they often have a brief half life, usually only need to pump and dump for 12 -24 hours
