A medication error is any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of healthcare professionals, patients, or consumers

- Skill based; familiar task , no conscious though, lack of attentional checks
- Slips: performing the wrong function
- Lapses: forgetting to perform a task
- Rule based: familiar task but not yet skilled, mistakes
- Knowledge based skills: unfamilar task, high conc levels, mistakes
