Mechanical ventilation Flashcards
mechanical ventilation purpose
maintain balance between CO2 and O2
FiO2 what is the cut off for not being a good candidate for PT
60 %
Bipap - non invasive ventilation
More pressure inhalation, less pressure exhalation
dog sticks head out window on inhale puts head back in car for exhale
regression from CPAP
CPAP: Continuous Positive Airway Pressure
dog sticks its out the window the whole time
Tracheostomy
Unable to be weaned from ventilator
Volume Controlled
Ventilator Modes
Preset tidal volume
constant or decelerated flow
volume drops means less compliant lung
volume increases with more compliant lung
Pressure Controlled
ventilator mode
pressure = preset
Inspiratory flow always decelerating
stiff lungs = HIGH pressure
compliant lung = LOWE PRESSURE
pros and cons of Intermit vent
pros more independence with breathing,
con= fatigue faster
cons and pros of constant vent
pros = can have pt. Exercise ,
con = no room for improvement ( no muscle atrophy so no strengthening)
PEEP does what
- Increases FRC
Pressure Support Ventilation (PSV) does what
- increase TIDAL VOLUME
Continuous Mandatory Ventilation (CMV)
versus Intermittent Mandatory Ventilation (IMV)
CMV = ventilator doing all the
work
IMV = can breath between mandatory breath from the machine
What is weaning/ liberation
decreasing or getting rid of ventilation all together
will start PT at this point
3 mechanical ventilation complications
- Skin breakdown (decubitus ulcers)
- Joint contractures
- Deconditioning
