Assignment
routine care, activities, and procedures that are within the authorized scope of practice
sharing of routine work
assigned tasks should be part of course work taught in basic edu program of the staff member
Delegation
allowing a specific task to be performed that is not routinely performed and is beyond the traditional role of the individual to which the nursing activity, skill, or procedure is delegated.
- must have received additional training or education
- must have shown competency
- you can delegate the responsibility of the task, but you cannot delegate the ultimate accountability
5 Rights of Delegation
right task right circumstances right person right direction & right communication right supervision & right evaluation
UAP
nursing assistant, scrub tech, clinical tech, etc.
can perform tasks on STABLE clients
they do routine, simple, repetitive, common activities that do not require nursing judgement
UAP cannot perform sterile procedures or give meds
no assessment (this includes addmission ht and wt bc its part of initial assessment)
LPN
no assessment or evaluation
can assist in data collection / updates client data
only does care for STABLE client
only RN can do admission history
can do treatments, medications, fingersticks, etc.
cannot develop plan of care (devise a teaching plan for client)
- they can do teaching, but its basic, standardized teaching or reinforced teaching
can’t push IV pain meds or blood products
Ex of unstable client
new admission
low blood sugar client
changes in routine VS (neuro checks / increasing BP)
- unstable vital signs
coming back from an invasive procedure
4 C’s of communication
clear
concise
correct
complete
provide this type of communication at the time of delegation and provide continued direction on ongoing basis
- need to give a time frame and the priority of the task
Supervision of Delegation
follow-up
- was the task done properly and in a safe manner?
- was the task done in a proper time frame?
- were the client’s needs met?
Social Media and Patient Privacy
no identifying factors EVER
only generic statements that can’t be tracked to anyone
no negative comments about facility, coworkers, or patients
