What are megaloblasts
RBC precursors
Immature nucleus (low B12/folate) with mature cytoplasm

Causes of B12 deficiency
Diet
Malabsorption - gastric (pernicious anaemia), ileal (coeliac, Crohn’s, tropical sprue), nitric oxide abuse
Clinical features of B12 deficiency
Mouth - glossitis, angula cheilosis
Neuropsych - mood/memory changes
Neuro - paraesthesia, peripheral neuropathy
Mechanism of pernicious anaemia
Autoimmune atrophic gastritis → achlorhydria and lack of gastric IF
Anti-parietal, Anti-IF
Mx of pernicious anaemia
IM hydroxycobalamin 6x in 2/52

Green vegetables
Folate deficiency causes
Poor diet
Pregnancy
Malabsorption
Drugs/alcohol
Drugs that can cause folate deficiency
Anti-epileptics (phenytoin)
Trimethoprim
Methotrexate
Folate deficiency treatment
Check B12 and replace if needed
oral folic acid
