Pathophysiology of paroxysmal nocturnal haemoglobinuria
Acquired loss of protective surface GPI markers on RBCs → complement mediated lysis → chronic intravascular haemolysis, especially at night
Sx and Dx of paroxysmal nocturnal haemoglobinuria
Sx - morning haemoglobinuria, thrombosis
Dx - immunophenotyping (altered GPI) or Ham’s test
Mx of paroxysmal nocturnal haemoglobinuria
Iron/folate supplements
Prophylactic vaccines/antibiotics
Eculizumab - prevents somplement binding to RBC
MAHA pathophysiology
Mechanical RBC destruction (formation of fibrin meshes in vessels) → schistocytes

HUS, TTP, DIC, pre-eclampsia, eclampsia
