What cancer is associated with inflammatory bowel disease?
MALT lymphoma ( a non hod lymphoma)
What is tumour lysis syndrome?
Cell breakdown after chemo leading to high levels of potassium, phosphate and Uric acid being released
Sx: acute renal failure, muscle cramps, confusion
Common in burkitts lymphoma
What disease is associated with starry star appearance on microscopy?
Burkitts lymphoma
What’s the most common type of Hodgkin lymphoma?
Nodular sclerosing (good prog, women, assoc with lacunar cells)
What’s the best prognosis Hodgkin lymphoma?
Lymphocyte predominant
Which is the worse prognosis Hodgkin lymphoma?
Lymphocyte depleted
What can be given to prevent/ reduce tumour lysis syndrome?
Allopurinol (inhibits xanthine oxidase and Uric acid production)
Rasburicase (urate oxidase turns uric acid to allantoin which is more easily excreted)
What’s the gene translocation in burkitts?
C-myc gene translocation
What’s the Anne Arbor staging system?
Used in lymphoma
1) located in a single region e.g. One LN.
2) two separate areas, both above or below the diaphragm
3) spread to both sides of the diaphragm
4) diffuse or disseminated involvement of one or more extralymphatic organs e.g. Liver, bone marrow or lungs.
Features of NHL lymphoma?
Nodal involvement (lymphadenopathy) ? Painful.
Extranodal - skin, MALT, orophraynx, small bowel,
Systemic - fever, night sweats, weight loss (⬆️ in HL)
Investigations for NHL lymphoma?
FBC, HIV/HEP screen, autoimmune, TFTs, U&E, LFT.
LN biopsy,
CT and functional PET-CT for staging (Ann arbor)
Treatment for NHL lymphoma?
Chemo, radiotherapy (for localised), and BM transplant,
Targeted - Rituximab (CD20) for b-cell disease
Presentation of HL lymphoma?
Teens and early 20s and elderly.
Enlarged non-tender rubbery LN, usually cervical.
Constitutional- fever, weight loss,
Hepatosplenomegaly
What is seen in biopsy for HL lymphoma?
Reed sternberg cells
What’s the treatment for HL lymphoma?
Chemo, radio, BM transplant
Brentuximab (CD30)
