What are the contraindications for lung transplant?
A
- Disease specific guidelines
- Old age
- Presence of concomitant disease significantly shortening life expectancy
- Contraindications to surgery/GA
- Active smoker, alcoholic
- Poor social support

What are the complications of lung transplant?
A
- Rejection
- Acute: lymphocytic inflammation
- Chronic: alloimmune inflammatory and non-alloimmune fibroproliferative, bronchiolitis obliterans - Graft dysfunction - reperfusion oedema in 1st week
- Airway complications - dehiscence, stenosis, bronchomalacia
- Infection - CMV, aspergillus, pneumonia

How do you manage patient post-lung transplant?
A
- Multidisciplinary approach, regular follow up, transplant coordinator
- ISTs - CNI, purine synthesis antagonist, steroids
- CMV and PCP prophylaxis
