What is fibrothorax?

A
  • Localised or diffuse irreversible fibrotic transformation of pleura
  • Pleural thickening > 5mm extending to major portions of hemithorax
  • Encases lung and restricting expansion, impairs respiratory function

What are the examination findings/signs of fibrothorax?

A
  1. +/- Trachea deviation towards affected side (in volume loss)
  2. Shrinking of hemithorax - reduced chest expansion over affected side
  3. Dullness to percussion
  4. Decreased vocal resonance
  5. Reduced breath sound but still present
  6. +/- pleural friction rub

A

Diffuse pleural thickening causes marked reduction in lung volume and hypoventilation of affected lung

A
  1. Lung function test - restrictive pattern
  2. CXR - malignancy or active PTB
  3. CT thorax - pleural thickening and fibrothorax
  4. Lung biopsy

How would you manage fibrothorax?

A
  1. Treat underlying cause
  2. Pulmonary rehabilitation
  3. VATS or open decortication

What are the indications for decortication surgery in fibrothorax?

A
  1. Significant respiratory symptoms
  2. Significant restriction vital capacity < 60%
  3. Trapped lung
  4. Chronic non-resolving empyema
  5. Failure of conservative treatment - antibiotics and pleural drainage of empyema

A
  1. Prolonged air leak and bronchopleural fistula
  2. Bleeding
  3. Post-operative infection
  4. Respiratory failure
  5. Failure of lung re-expansion
